This program is an option for families who earn less than 100% of the federal poverty guidelines and who are in need of primary care or urgent care services in our community.

PrimaCare Program
The PrimaCare Program is a partnership between Collin County Health Care Services, Prima Care medical centers and Farmersville Family Clinic.  This program is an option for families who earn less than 100% of the federal poverty guidelines and who are in need of primary care or urgent care services in our community. Family-friendly office hours, a variety of services and locations throughout the county are just some of the options available to patients who qualify. Application forms are provided at the locations listed below. Eligibility: To determine if someone is eligible to receive medical assistance through this program, the household must earn less than 100% of the federal poverty guidelines. Please see the Links & Resources section at the bottom of this page for federal poverty level information.

  • Prima Care: Offers primary care, urgent care, and occupational medicine services seven days a week; no appointment necessary. 5 locations in Collin County, and program’s website will help you look up the one nearest you -- or you can download a document of PrimaCare locations and hours.
     
  • Farmersville Family Clinic: Offers family practice, general medicine, pediatrics, women’s health, diabetes and nutrition education and consultation. Open Monday-Thursday, 8-a.m.-5 p.m. 111 N. Johnson St., Farmersville, TX (map link); (972)782-6131.
